Advance Care Directive
We help you create a legally binding Advance Care Directive and designate Medical Treatment Decision Makers, giving you a clear way to communicate your wishes.
This documentation can be shared with your family, friends, medical team, uploaded to 'My Health Record,' and provided to legal representatives, or anyone else you choose.
This service is available as a stand-alone option and is particularly beneficial for clients entering an Aged Care Home or similar settings.
