Advance Care Directive
We can assist you in preparing a legally binding Advance Care Directive and in nominating Medical Treatment Decision Makers. This gives you a clear and thoughtful way to express your medical preferences ahead of time.

Your completed Directive can be shared with family, friends, your healthcare team, uploaded to your My Health Record, or provided to legal representatives, so your wishes are known and respected.

This service is available on its own and is particularly helpful for those entering aged care or similar settings.
